Changing Your Legal Name

You must file a formal request with your county’s superior court to change your name in Arizona.

Procedures, forms to be filed, supporting documentation, and fees vary by court.

If the court approves your name change, you will receive a court order that can be used to update the name on your ID documents.

Arizona Generic Name Change Form

Submit a request to Arizona’s Bureau of Vital Records to amend your birth certificate.

To change the name on your Arizona birth certificate, you will need to provide:

  • complete birth certificate request form (PDF)
  • completed and signed affidavit to correct birth certificate (PDF)
  • certified copy of name-change court order
  • copy of front and back of your signed government-issued photo ID (not needed if your application signature was witnessed by a notary)
  • payment of applicable fee(s)

Note: You must update your birth certificate with the state where you were born.

You must update your name with the Social Security Administration at least two days before you can change your name on a Arizona-issued ID.

You need to visit the MVD or another authorized office to update your license or ID within ten (10) days of changing your legal name.

Bring a certified copy of your name-change court order or other legal document demonstrating your legal name change.


Updating Gender Markers

The Bureau of Vital Records will amend the gender marker on your Arizona birth certificate if you meet specific medical requirements.

The state recommends that you contact the Bureau of Vital Records to confirm what will be required for your case.

To apply for an updated Arizona birth certificate, you will need to provide:

  • completed and signed affidavit to correct birth certificate (PDF)
  • completed birth certificate request form (PDF)
  • notarized letter from physician that confirms you meet the state’s medical requirements
  • copy of front and back of your signed government-issued photo ID (not needed if your application signature was witnessed by a notary)
  • payment of applicable fee(s)

Note: You must update your birth certificate with the state where you were born.

Visit the MVD or an authorized office to update the gender marker on your Arizona driver’s license or state ID.

You will need to provide a signed physician’s statement that certifies you are “irrevocably committed” to the transition process.
